FOOTBALL/CHEERLEADING REGISTRATION

REQUIRED DOCUMENTS

  1. Signed and completed registration form and Parent/Guardian Consent form.

  2. Paid Registration Fee of $60 (CASH ONLY)**

  3. Copy of Birth Certificate (No School Forms will be accepted)

  4. Final Report Card (85 pound and up only)***

  5. Current Picture (Mug Shot)

  6. Physical Fitness Form (form has to be signed by Doctor on required form)

  7. Medical Consent Form

  8. Participant Insurance Fee of $6.00 (optional)

**EQUIPMENT/UNIFORMS WILL BE ISSURED WHEN REGISTRATION FEE IS PAID IN FULL

**NO REFUNDS AFTER AUGUST 18, 2006

***Must register for Tutoring/Homework Assistance Program if grade point average is less than "2.00"
***Must sign consent form for weekly Progress Report form participants' school.
